What is a perineal hernia repair?

A perineal hernia occurs when the muscles of the pelvic floor (the perineum) weaken or separate, allowing abdominal contents—such as fat, the rectum, or even the bladder—to bulge into the area surrounding the anus. This condition is most commonly seen in intact, middle-aged to older male dogs.

Recognising the signs

Perineal hernias typically develop slowly. If you notice any of the following, your pet should be evaluated by a veterinarian:

Swelling

A soft lump on one or both sides of the anus.

Straining

Noticeable difficulty or pain while defecating (tenesmus).

Constipation

Infrequent or incomplete bowel movements.

Altered Tail Carriage

The tail may be held differently due to discomfort.

Urinary Emergency

If the bladder becomes trapped in the hernia, your pet may be unable to urinate—this is a life-threatening emergency.

The surgical procedure

Internal Obturator Transposition:

This is the gold standard technique. We carefully reposition a nearby muscle (the internal obturator) to "bridge the gap" and create a strong new pelvic floor.

Concurrent Neutering

Since hormones play a significant role in the weakening of these muscles, we perform a castration (neutering) at the same time as the repair to drastically reduce the risk of recurrence.

Post-operative care & recovery

Recovery is a collaborative effort between our clinical team and you at home. Most patients enjoy a high success rate with proper aftercare.

  • Pain Control - Multi-modal pain relief including anti-inflammatories and analgesics.
  • Activity Level - Strict rest for 2–3 weeks; short, on the lead toilet breaks only.
  • Incision Care - Use of a buster collar to prevent licking and frequent monitoring of the site.
